HELP: HTML form

Call me stupid... I don't know exactly what it's called, but would anybody be willing to give me a step-by-step on how to create a form for a website in html and have the information sent to an email? like some clan applications.

(in dreamweaver preferably, but code would work as well)

If your using Dreamweaver there should be a tutorial on how to submit forms somewhere in the dreamweaver extras. Or just look it up in the thick book that comes with the legit copy of Dreamweaver.
